A criação de um fluxograma para diagnóstico e tratamento

Clinicamente, a colecistite aguda manifesta-se como dor no quadrante superior direito do abdômen, febre, náuseas e vômitos, que podem ser exacerbados pela alimentação. O diagnóstico é realizado por meio de exames clínicos, laboratoriais e de imagem, sendo a ultrassonografia o exame inicial de escolha, com a cintilografia hepatobiliar indicada em casos em que a ultrassonografia não é conclusiva. O tratamento, na maioria dos casos, consiste em colecistectomia laparoscópica precoce, preferencialmente realizada dentro de 72 horas após o diagnóstico, para evitar complicações graves como gangrena e perfuração da vesícula biliar. Este estudo propôs a criação de um fluxograma clínico para o manejo da colecistite aguda, fundamentado nas Tokyo Guidelines 2018 e entrevistas com especialistas. O objetivo foi padronizar e agilizar o processo diagnóstico e terapêutico, desde a suspeita clínica inicial até a definição das estratégias terapêuticas, com base na estratificação da gravidade do quadro clínico.
